Information on Data entry for Quality use of Medicine database usersBefore entering data into this database you must register to apply for a user name and password. Please complete all fields for contact details. After registering, a password and user name will be supplied. To enter data, log in and choose Manage/Add Projects and then complete your project details. Explanation of Project Information fieldsPlease Note: The first four fields are required fields.INFORMATION CATEGORY I: GENERAL Project Title: Enter an appropriate title for your QUM project Short Title: Enter a brief title for your QUM project Abstract: Present a short, succinct summary outlining your project/program. The description should include aims, methodology, results or expected outcomes. If you are describing an organization, please include the terms of reference. Report Description: Describe the format of your report here. For example, Conference Abstract, Technical Report, Statutory Report, etc. Other contacts: Enter details of other people associated with the project if needed. Report url: If your report is published on the web, enter the appropriate url here. Project Owned By The Australian Government: Please indicate here whether the Australian Government owns the Intellectual Property from your project. Project Completion Stage: Specify if your project is planned, current, completed or ongoing. Start date: Date project was or will be started Completion date: Date project was or will be completed Key words: Enter words that describe your project. This will help people searching the database to find your project if it is relevant to them. INFORMATION CATEGORY II: FUNDING Funding amount: Enter funding amount ($), preferably in numeric form with no commas. Funding Source: Select funding source(s). Provide details for Non-government, Industry, or Other (if possible) in the adjacent text boxes. INFORMATION CATEGORY III: PROJECT SCALE AND LOCATION Project Location: Click on the Longitude, Latitude or Radius boxes to bring up a map of Australia. Zoom in as necessary before clicking on the location at which your project was conducted. Select a radius from the drop-down menu to indicate the reach of the project. Project Implications: Indicate by selection from the drop-down list whether you think your project has international, national, regional or local implications. Project Scale: Indicate by selection from the drop-down list whether you think your project has Worldwide, nationwide, statewide, regional, city or town level scale. GP Division(s): This field identifies projects that have involved Divisions of General Practice. Select the Division by scrolling through the list and clicking on the appropriate entry. If you need to select more than one division, hold down the Ctrl key (or Command key or equivalent) and click to make the other selections. INFORMATION CATEGORY IV: PROJECT TARGET GROUPS Target Groups: Identify the group or groups at which your project is aimed. One project can have one or more target group. If you select 'Other health professionals', 'Industry' or 'Other', please provide additional details in the adjacent text box. If your project does not target any specific groups, please check the 'Not Applicable' box only. INFORMATION CATEGORY V: PROJECT CLASSIFICATION Project Classification: Select classifications which describe the most appropriate subject area or areas of the project. If your project does not fit under any of the listed classifications, or can be classified in other ways, select Other box and enter a classification description in the adjacent text box. INFORMATION CATEGORY VI: PROJECT INVOLVEMENT Project Involvement: Select which groups were actually involved in the design and implementation of the project. If possible, please give details for Other health professionals, Industry and Other groups in the adjacent text boxes. INFORMATION CATEGORY VII: QUM RESOURCES QUM Resources: Indicated if the project resulted in the development of any QUM resources - please provide details in the adjacent text boxes. INFORMATION CATEGORY VIII: PROJECT SETTING Project Setting: Select one or more options to describe where the project was conducted. If none of the options fit your project setting, select Other and enter a brief description of the setting in the adjacent text box. INFORMATION CATEGORY IX: PUBLIC REPORTING Public reporting: If your project has resulted in the production of any Conference papers, Journal articles, Newsletters, Posters, Web sites or any other types of publicity, please indicate by checking the relevant boxes. For Other, please provide a brief description in the adjacent text box. Publication Details: Please provide details of any publications indicated by your selections in the Public Reporting categories so that people searching the database can locate the publication, or view a website if they are interested. For example, please provide names of authors, title, journal, date, volume, page numbers, publisher, or web address if appropriate. INFORMATION CATEGORY X: EVALUATION MECHANISMS Evaluation mechanisms: Please indicate if your project has been evaluated, or if this is planned. The categories are : Not planned, To be completed, Done, and Not applicable. Also select the type of evaluation (process, impact, outcome) if applicable. The types are defined as follows.
INFORMATION CATEGORY XI: EVALUATION METHODOLOGY Evaluation methodology: Select all that apply by checking the relevant boxes. The categories are Pre-post test (single group), Pre-post (control group), Randomised controlled trial, Single group (time series), Non-equivalent group (time series), and Other. As before, for Other, please provide a description of the evaluation methodology used in the adjacent text box. To enter your project, click on the Add button (please check project before updating the database). Note: All information supplied will be moderated before the project will included on the database.
